Overview: The Successful Transitions© Program at the San Diego Center for Children supports at-risk youth aged 15 to 18 in navigating the challenges of transitioning to adulthood. Designed for youth with behavioral health challenges, learning differences, developmental disabilities, or a history of trauma, the program helps participants create personalized plans and build skills for success in adult life. This program includes a collective-impact approach, uniting providers, agencies, and nonprofits to drive meaningful change in how at-risk youth are supported.
